Dr. Gleason is an Associate Professor of Medicine at UChicago Medicine in the Section of Geriatrics and Palliative Medicine. She is a former HRSA-funded Geriatric Academic Career Award (GACA) recipient and currently serves as a co-investigator on a HRSA-funded Geriatrics Workforce Enhancement Program (GWEP), where she leads nursing home educational efforts and Age-Friendly Health Systems initiatives. Her work focuses on implementing age-friendly care models across the post-acute continuum. She has led numerous Project ECHO sessions educating nursing-home staff, CNAs, social workers and published her findings. Dr. Gleason serves on the PALTmed Clinical Care Committee.
