Session Summary: This session shares lessons from integrating consulting palliative Nurse Practitioners into nursing homes, focusing on early identification, de-stigmatizing “palliative care,” and strengthening interdisciplinary communication. Presenters will review clinical triggers (e.g., weight loss, cognitive decline, hospitalization), introduce structured communication tools for use with patients and families—such as Ask-Tell-Ask, REMAP, and “I wish/I worry”—and highlight strategies including staff/provider education via an educational video and in-person discussions with Nurse Practitioner consultants, plus a hospice review board to guide appropriate referrals. Attendees will engage in case-based discussion and leave with practical, evidence-based tools to improve palliative and hospice access.

At the completion of this session, learners will be able to:
Identify three clinical indicators for palliative or hospice referral in nursing home residents and describe strategies to counter misconceptions that palliative care is only for end-of-life.
Apply at least two structured communication tools—such as Ask-Tell-Ask, REMAP, or “I wish/I worry”—to goals-of-care discussions with patients and families.
Outline two systems-level strategies for integration, including consulting NP roles, hospice review boards, targeted staff/provider education, and improved EMR documentation of advance directive conversations.
