Carl J. “Christian” Bergman, MD, CMD, FACP, AGSF is an associate professor of Geriatric Medicine at Virginia Commonwealth University in Richmond, VA. His clinical practice focuses on post-acute and long-term care medicine with active medical directorship in 2 buildings. He is focusing on improving transitions of care, teaching the next generation of physicians, and focusing on patient advocacy for older adults in the community. At PALTmed, he serves on the Board of Director as Chair of the House of Delegates. He is past chair of the State Based Policy and Advocacy Subcommittee and past chair of the Transitions of Care subcommittee.
