Session Summary: As the post-acute landscape evolves toward value-based care, physician practice groups are being asked to demonstrate not only their clinical excellence, but also their role in driving outcomes, reducing costs, and improving care coordination. Yet many practice groups lack the visibility, data, and tools needed to tell that story, especially in skilled nursing and assisted living settings where care fragmentation and partial coverage are common. In this interactive session, we will explore how physician groups can harness real-world data to expand both their patient coverage and enrollment in risk-bearing programs like Institutional Special Needs Plans (I-SNPs), D-SNPs, C-SNPs, and ACOs.
