Session Summary: As the post-acute landscape evolves toward value-based care, physician practice groups are being asked to demonstrate not only their clinical excellence, but also their role in driving outcomes, reducing costs, and improving care coordination. Yet many practice groups lack the visibility, data, and tools needed to tell that story, especially in skilled nursing and assisted living settings where care fragmentation and partial coverage are common.
In this interactive session, we will explore how physician groups can harness real-world data to expand both their patient coverage and enrollment in risk-bearing programs like Institutional Special Needs Plans (I-SNPs), D-SNPs, C-SNPs, and ACOs.

Learning Objectives:

At the completion of this session, learners will be able to:
  • Understand how physician-led care impacts key value-based metrics. Participants will gain clarity on how physician practice groups influence hospitalization rates, documentation quality, medication appropriateness, and chronic disease outcomes in post-acute care—and how these outcomes translate into financial and clinical value.
  • Identify strategies to expand coverage and enrollment within partially covered facilities. Attendees will learn how to leverage real-world data to grow their footprint in skilled nursing and assisted living environments, enhance attribution accuracy, and drive enrollment into aligned risk-bearing entities like I-SNPs, C-SNPs, D-SNPs, and ACOs.
  • Gain insight into the product development lifecycle and how to guide vendors toward purpose-built solutions.
